Craig i had natural ability when i played rugby,but didnae train very well and drank too much

Craig, appreciate if you can share some insights, as a trainer.
I often see "cast a shoe in stipe comments".
What causes this to happen, during running of the race?
If a horse is fitted with shoes, and carded to run with shoes, do they train with them as well?
Should something happen to a horse or rider during the running of a race, can the course manager be held accountable, if it is due to the course condition, or is the ferrier to blame, for a shoe been cast, or is the horse to blame?
No sarcasm intended, just require some enlightenment.
It is kind of like, fitting a tyre onto a car and not ensuring that all the nuts that secure the tyre, are secure.

Would be interesting to know what Randolps riding instruction were on Masked Lady.
